I wonder what would happen if you didn't have a patella (knee cap)

  1. There is actually a rare genetic disorder called nail-patella syndrome in which the patellas are absent or under-developed, along with other deformities or malformations to the musculoskeletal system and bladder and kidney system.

The absent or reduced patellas causes knock knee deformity, or lateral shift on knee flexion. This condition can also be caused by osteoarthritis and similar conditions.
